VACANCY: Armed Forces Community Social Prescribing Link Worker

We’re recruiting an Armed Forces Community Social Prescribing Link Worker (AFC SPLW)! The role is to provide additional support for the armed forces and veteran communities. The AFC SPLW will act as a resource for receiving referrals and supporting those within the AFC.

The project will be part of a ‘Test and Learn’ Demonstrator, which aims to scope out how social prescribing can support the specific needs of the Armed Forces Community.

You will be responsible for providing personalised support to individuals, their families and carers to take control of their health and wellbeing, and develop trusting relationships. You will also work with other Social Prescribing Links Workers to train and educate them on the needs of the AFC and how Social Prescribing can provide valuable support.

This is a One Year Fixed Contract, funded by Cornwall Council.
