Armed Forces Community Social Prescribing Link Worker

Armed Forces Community Social Prescribing is a specialist service to help address a range of health needs and inequalities experienced by the Armed Forces Community. As an Armed Forces Community Social Prescribing Link Worker you will provide signposting, advocacy and support across Cornwall and the Isles of Scilly linking in with individuals, professionals and Primary Care Networks.
